Job Description

Transitions of Care (TOC) is one of Devoted Medical’s largest clinical programs, responsible for ensuring that thousands of Devoted members return home safely after hospitalization. From admission through 30 days post-discharge, our highly skilled, virtual interdisciplinary teams deliver personalized and comprehensive ‘wrap around care’ to break down the gamut of clinical, social & logistical barriers to post-discharge stability. This member-centric approach has already yielded substantial, sustained reductions in post-discharge complications and readmissions, and we’re continuously evolving our care model to keep more members safe at home.
That’s where you come in. We are looking for a seasoned, conscientious, and transformational clinical leader and thought partner, a scrappy, empathetic, determined individual with a proven track record in leading high performing clinical teams to post-acute patient success. Transitions of Care’s Senior Manager will lead a high performing team of nurse care managers to provide virtual clinical care for a wide range of diseases and across a wide spectrum of acuity. Partnering closely with TOC’s operational & clinical leaders, the Senior Manager will help build a care model and manage a workforce of nurses and unlicensed care coordination staff to provide excellent care in the 30-day window after patients leave the hospital. The Senior Manager will develop and inspire a team of managers to lead engaged, quality-oriented teams that obsess over caring for patients like family and help them stay healthy at home.
Required skills and experience:
5+ years of experience leading high performing clinical teams in a transitional care and/or care management setting, ideally across a virtual and/or geographically dispersed workforce
3+ years of experience in coaching and developing other leaders/managers (i.e. past manager-of-managers)
Active RN license in good standing, and/or BSN or equivalent related degree, master’s preferred
Experience in post-acute medicine, nursing and case management processes, internal medicine, urgent care, emergency room, hospital medicine, and/or geriatrics
Experience managing and/or working in an interdisciplinary care team setting, along with a strong conviction for interdisciplinary care in general
Extensive experience as an enthusiastic change agent/transformational leader

Certification in Case Management a plus!
Your Responsibilities and Impact will include:
Transformational Clinician Leadership: Cultivate & inspire a collaborative, patient-oriented, high-performing team of front-line nurses, care coordinators, educators & nurse leaders dedicated to improving post-acute patient experiences & outcomes
Coaching & Development: Drive hiring, training, workflow improvement, skill development, and performance management strategies for frontline teams and managers
Results-Driven Performance Management: Track & manage staff performance against a combination of productivity, quality of care and clinical outcome KPIs to ultimately reduce avoidable admissions & readmissions; lead regular meetings with managers & front-line staff to provide feedback, identify & act upon opportunities for improvement, and provide consistent coaching & encouragement
Thought Partnership: Work closely with other clinical & operational program leads to shape the program’s scope, strategy & execution to optimize outcomes for our patients & our interdisciplinary care teams
Salary range: $105,000 - $174,000 annually
Our ranges are purposefully broad to allow for growth within the role over time. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered may depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
