A bit about this role:

We want to help patients navigate the healthcare system in a better and safer way, feel their best, and better manage their chronic diseases. The Associate Supervisor leads a team of TOC Associates who support patients during one of the most vulnerable moments in their healthcare journey, transitioning home after a hospital stay. As a supervisor, your mission is to ensure your team delivers safe, timely, high-quality support that prevents unnecessary readmissions and creates a compassionate, family-like experience for every patient.

You will coach, develop, and support your team while ensuring operational excellence, consistent workflow execution, and strong collaboration with clinical partners. You’ll help shape a culture of accountability, empathy, continuous improvement, and member-centered care within Devoted’s post-acute ecosystem.

Our ideal Supervisor is organized, mission-driven, proactive, and energized by developing people, improving processes, and advocating for members during critical care transitions.

Responsibilities will include :

  • Lead, coach, and mentor a team of Transitions of Care Associates to achieve performance, quality, and patient experience goals.

  • 30% of the time will be dedicated to providing TOC patient care. Time spent on clinical responsibilities will fluctuate depending on the number of direct reports.

  • Conduct regular 1:1s, team meetings, and coaching sessions to support skill development and engagement.

  • Create a supportive, inclusive, high-performing team culture grounded in Devoted’s values.

  • Provide timely feedback and performance management, escalating concerns when appropriate.

  • Support onboarding, training, and competency development for new team members.

  • Ensure team adherence to TOC workflows, documentation standards, and escalation pathways.

  • Monitor daily, weekly, and monthly key performance indicators (KPIs)

  • Use data to identify trends, barriers, and opportunities for improvement.

  • Partner with leadership on staffing needs, workload balancing, and scheduling.

  • Support your team in ensuring timely outreach to patients after hospital discharge.

  • Guide associates on navigating complex cases

  • Supports rollout of updates, training refreshers, and change-management initiatives to improve member outcomes

Desired skills and experience:

  • Medical Assistant Certification or LPN Required

  • Previous supervisory experience highly desired

  • Fully bilingual in English and Spanish strongly preferred.

  • Experience working in a remote, high-volume, fast-paced environment.

  • Experience with transitional care, post-acute workflows, or care coordination.

  • A history of being organized and having high attention to detail - it’s the little things that matter

Salary Range: $60,000 - $70,000 annually

The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
