A bit about this role:
We want to help members navigate the healthcare system in a better and safer way, feel their best, and better manage their chronic diseases. Our transitions of care program is critical to achieving these goals, and TOC coordinators will play a key role in supporting these programs.
As the supervisor for the TOC coordinators, you'll lead a team of TOC coordinators in serving as advocates for these members, supporting the TOC clinical team. The goal is to ensure the members have the necessary resources and support to transition to home from the hospital and/or post acute setting and prevent readmissions.
The supervisor will manage a team of TOC coordinators and guide their work to take care of members during the transition of care episode: connecting with members; enabling ancillary services like appointments, transportation, supplies, and durable medical equipment; and follow-up communication.The TOC supervisor is an integral part of the TOC team, and will be heavily involved in the day to day team operations, as well as being an active participant in rounds/huddles. The supervisor will provide support to the team, develop new workflows and job aids, coach and manage performance, and attend and/or facilitate regular meetings with team members and leadership as required.
Responsibilities will include :
30% of the time will be dedicated to providing TOC patient care. Time spent on clinical responsibilities will fluctuate depending on the number of direct reports.
Assists as needed with monitoring team attendance and adherence and participation in meetings/forums
Collaborating with leadership in TOC to help develop the program
Join cross functional meetings to represent TOC coordination and share relevant updates with the larger team
May be assigned additional responsibilities as the TOC care model evolves
Assist in recruiting and hiring process of new TOC Coordinators
Participate and coordinate TOC Coordinators involved in TOC clinical model pilots
Partner with the TOC trainer and DM education team in onboarding new TOC Coordinators
Manage TOC coordinator performance across array of key performance indicators
Provide oversight and support to the TOC Coordinators by answering questions, monitoring relevant slack channels and conducting 1:1s
Process improvement: assist in design, & implementation of new workflows, including job aid creation and facilitating trainings
Meets with Senior Manager regularly to go over team performance and make recommendations for improvement

Desired skills and experience:
Minimum educational experience: high school diploma or GED equivalent
Associate’s degree in health related field a plus
Certification in healthcare a plus (social services, LPN, MA)
Previous supervisory experience highly desired
2+ years healthcare administrative/office experience Proficient in technology using evolving platforms
A history of being organized and having high attention to detail - it’s the little things that matter
Fully bilingual (English/Spanish) preferred but not required
Salary Range: $60,000 - $70,000 annually

The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
